Intake, Practice tools
Considerations when responding to information received
Use these prompts to gather and assess as much information as possible from the person reporting the concerns about te tamaiti.
Assessment and planning
Using chronologies to support decision-making in the initial assessment phase
A chronology records the key life events of tamariki or rangatahi and their whānau or family to help us understand their journey to date and identify patterns of harmful impacts on oranga or behaviour and times when oranga has been positively experienced.
Assessment and planning
Seeking consent to talk to tamariki and rangatahi
Wherever practicable, we seek consent from a parent or guardian when we need to talk directly with tamariki or rangatahi aged under 18 years old.
Practice tools
Gateway assessments
A gateway assessment is an interagency process that helps to clarify and identify ways to address the health and education needs of tamariki and rangatahi we work with.
Assessment and planning
Initial assessment phase
We gather quality information and develop a chronology to build understanding of oranga. This supports decision-making about the best response and appropriate level of support for te tamaiti or rangatahi and their whānau or family.
Care, Policy
Support for tamariki and rangatahi in care or custody
This policy outlines our support obligations to address the needs of tamariki and rangatahi in the care or custody of the chief executive.
Care
Visits with tamariki and rangatahi in care or custody
We need to visit tamariki and rangatahi as often as they need us to so they can participate and influence decisions about their safety, care and wellbeing needs.
Policy
Casework responsibilities when our work involves more than 1 site
When our casework involves more than 1 site, we must uphold the right of tamariki, rangatahi, their whānau or family, and caregivers to a continuous and easily accessible social work service, where all relevant information is communicated.
Assessment and planning
Core assessment phase
The core phase of assessment builds on the initial phase to deepen our understanding of oranga, establishing a picture of the safety, strengths, vulnerability and needs of te tamaiti or rangatahi and their whānau or family.
Care
Caregiver support plan
We help caregivers to identify any support or training they need so they can meet the needs of tamariki and rangatahi. We create a support plan to document this additional support.
